Click on the link below to access our Process Material Cert. and Return Evaluation PDF.  Fill out the PDF (the fields outlined in RED are required), click on the Customer Signature tab to digitally sign it, and then click email form to email it to our returns team. Don't forget to click Save to save your completed form.

Not using Microsoft Explorer

Click on the link below to access our Process Material Cert. and Return Evaluation PDF. Download and save the PDF. Open the saved PDF, fill it out (the fields outlined in RED are required), click on the Customer Signature tab to digitally sign it, save the completed document, and then click email form to email it to our returns team.

They will respond to you within 24 hours with a return number and instructions on how to return the product(s) or assembly(ies). 

Process Material Cert. and Return Evaluation Form

** By Federal law, we are not permitted to handle a product without a Material Safety Data Sheet for the material used with the product. This includes any material that has contacted the product, including a gas or liquid used to test the product during installation or before operation. To ensure the safety of all associates handling these products, we cannot evaluate a contaminated product or a product that contains or was exposed to an unidentified material. In addition, we need to know what fluids were used in the product so that we can provide a useful evaluation. Compatibility of the process fluid with the product’s materials of construction can affect product performance.
